Office hours are 7 a.m. to 5 p.m., Monday through Friday. For more information about the UCIA program, call (815) 740-5160. Electronic Conviction Inquiries - The Illinois State Police Bureau of Identification has developed a program that will allow individuals and/or agencies to process name-based Uniform Conviction Information Act (UCIA) background checks electronically via email and receive a response within approximately 24 hours. This program is referred to as FEESUB. To have your criminal conviction expunged, visit the Office of the State Appellate Defender website. They have published information for individuals seeking to have criminal records expunged at http://www.state.il.us/defender/exp.html. [ Back to top ] 3.) Where do I file a consumer complaint? If you've been victimized by fraud, deception, or unfair methods of competition, the Consumer Protection Division of the Illinois Attorney General's office may be able to help. Although their attorneys cannot represent an individual in a lawsuit, their limited mediation program can assist in resolving disputes or complaints filed by individual consumers. Promptness in making a complaint decreases the likelihood that a fly-by-night operator will move, leave town, or go out of business before consumers can gain assistance. For your convenience, the Office of the Illinois Attorney General Lisa Madigan has posted downloadable complaint forms on their website. These forms may be downloaded and returned by regular mail to the appropriate bureau within the Office of the Illinois Attorney General Lisa Madigan. To ensure that your form is processed efficiently, carefully review the form you have downloaded, supply all information requested, and include supporting documentation. The information you submit will be used by attorneys, investigators, and other members of the Attorney General's staff involved in carrying out the functions, programs, and responsibilities of the Illinois Attorney General. If you fill out a consumer complaint form, they may forward a copy of your complaint to the person or entity you are complaining about. This information may also be shared with other governmental enforcement agencies responsible for consumer protection and other laws. Complaint forms are public records, which are subject to the Freedom of Information Act just like other public records. If we receive a request for copies of records that include your complaint, they may be required to provide a copy of your complaint to the requestor. Information that would reveal your identity as a complainant is, however, exempt from disclosure and will be deleted from copies that we provide, allowing the requestor to read your complaint without compromising your privacy. An Adobe Acrobat Reader is needed to view and print PDF documents available from their network or at http://www.adobe.com/products/acrobat/readstep.html. Directions: Download, print and fill out a consumer complaint form (PDF) at http://www.ag.state.il.us/consumers/conscomp.pdf or consumer complaint form en Español (PDF) at http://www.ag.state.il.us/espanol/spancnsm0901.pdf. Please include your complete address and phone number as well as the business' address and phone number.
